REHABILITATION CENTER -

 

1.         How do I pay for this service? - There are several different options for payment.  Some people have private insurance that can help.  Some people pay privately.  Medicaid pays when the resident qualifies financially and medically.  Medicare only pays after a hospital stay of 3 consecutive, full days and if a skilled need exists.  If Medicare is the designated payment option, check with the facility for details.

 

2.         What items do I bring for him/her? - The room is furnished with a bed, a night stand, a chair and closet space.  We recommend that the Resident bring at least 5 changes of clothes, pictures to hang on the wall, bed coverings that are familiar, and basically anything that will make the Resident feel more at-home.   The room has limited space, so if you wish to bring furniture items, you should check with the admissions coordinator and/or social services to ensure they will fit.

 

3.         How do I convince them they need to come? - This can be a difficult issue.  Resident adjustment is always better if he/she chooses to come.  This is often not the case.  Sometimes it is helpful to have the physician tell your loved one they need additional nursing care.  If necessary, and as appropriate, you can seek guardianship.  This allows you to make decision for your loved one.

 

4.         Do people admitted to the Rehab Center ever leave and go home? - It is not uncommon for people to leave the facility and go to a less restrictive environment.  Currently, approximately 46% of our Residents go home after rehabilitation.

 

5.         What is the level of staffing that will care for my loved one? - Most often the ratio of Certified Nursing Assistants to Residents is 1:8 or 9.  We also have licensed nurses, licensed social service workers, recreation therapy staff, music therapy, and restorative and rehabilitative staff.

 

6.         What activities are available? - We have a full activity staff who focus on the quality of life for each individual resident.  We have daily activities of an abundant variety.  This staff not only focuses on group activities, but they work with each resident according to his/her needs.  This includes 1:1 interactions.

 

7.                   What are your visiting hours? - We have open visiting hours.  That means you can visit at any time and for as long as you like.  For more information, you can speak with our admissions coordinator and/or the social service worker.

 

8.                   What is the room availability?  - We generally have rooms available.  In the event that we do not, rooms open up frequently.  We can accommodate most people very quickly.

 

9.                   Do you provide respite services?  - We generally can accommodate the needs of those requiring respite services.  For more information, you can contact our admissions coordinator.

 

10.               What does your daily rate include?  - Our daily rate includes the room, the services provided by the various staff who work here, utilities (except for the telephone), meals, snacks, activities, housekeeping/laundry services.  Other costs are handled individually based on payment sources.  You can check with our admissions coordinator for more information.

 

11.               Do you provide transportation services?  - Transportation services are provided.  There is generally a small additional charge for this service. 

 

12.               Is there an age range for those people you admit?  - We do not have an age range for those eligible for admission.  We have had infants up to the very elderly as residents in our facility.  We have quite a number of young residents who live here on a regular basis.

TERRACE GROVE ASSISTED LIVING -

 

1.         How do I pay for this service? - There are some people who have long term care insurance that will pay for this service.  Most people pay privately.  We do have two apartments where the rent is based on a sliding scale.  We are happy to discuss these options with you.

 

2.         What items do I bring for him/her? - The apartment is unfurnished.  The reason for this is that most people prefer to bring their personal items with them.  We do not have a problem with pictures being hung on the walls.  The apartments are spacious enough to accommodate a fair amount of furniture that the resident wishes to bring from home.  We have floor plans available to assist you with mapping out which pieces of furniture you wish to bring and where you will be able to put them.

 

3.         How do I convince them they need to come? - This can be a difficult issue.  Resident adjustment is always better if he/she chooses to come.  This is often not the case.  Sometimes it is helpful to have the physician tell your loved one they need additional care.  If necessary, and as appropriate, you can seek guardianship.  This allows you to make decision for your loved one.

16.               What does your daily rate include?  - Our daily rate includes the apartment, the services provided by the various staff who work here, utilities (except for the telephone), meals, snacks, activities, housekeeping/laundry services.  You will likely need to pay for medications, supplies, physician visits, transportation services, etc.

 

17.               Do you provide transportation services?  - Transportation services are provided.  There is generally a small additional charge for this service. 

 

 

18.               What is the criteria for admission to assisted living? - The person who is admitted to a Type II assisted living facility must meet the following criteria: 

 

$                    He/She must not have a communicable disease that cannot be treated on an outpatient basis.

$                    He/She must not be a danger to self or others.

$                    He/She must be able to evacuate the building with no more than the assistance of one person.  If the individual requires a wheelchair most of the time, he/she must be able to pivot and transfer with no more than the assistance of one person.  He/she must also be able to propel themselves down the hall.

$                    They cannot have a skilled need.  If there is a question regarding this, a variance can be applied for by the facility to State of Utah, Bureau of Health Facility Licensure.
