Adult Day Center Charges

Effective November 1, 2006

 

Rates other than the standard Level I charges listed below are based on the individual’s needs as assessed by the Director and Nurse of the Day Center.  (See Level Rate Chart and Level of Care Assessment forms attached.)

 

LEVEL I RATES:

• $38.00 per day for individuals living at home
• $34.00 per ½ day for individuals living at home
• $32.00 for a full day session for Rehabilitation Center or Terrace Grove residents
• $26.00 per ½ day for Rehabilitation Center or Terrace Grove residents

 

CHARGES FOR ANCILLARY SERVICES (for a Day Center client who lives at home)

• Whirlpool/shower                                             $ 8.00
• Restorative Nursing Services                             $ 4.50
• Transportation                                     

                        Round trip within 5 miles                       $10.00   ($5.00 each way)

                        Round trip within 11 miles                     $12.50   ($6.25 each way)

                        Round trip 12 or more miles                  $15.00   ($7.50 each way)

                        Round trip over 22 in Cache Valley       $20.00    ($10.00) each way)

                        Brigham City                                         $30.00

                        Ogden                                                   $50.00

                        Salt Lake City                                        $75.00

 

• Overnight stay in the center.  Scheduled through Terrace Grove or ADC administrative personnel.    $90.00
• Care in the center beyond normal hours

                        Early morning care by appointment        $7.50/ half hour

                        After hours care by appointment            $7.50/ half hour   

                        Late fee – no appointment                     $3.75/ 15 minutes

• Beauty shop charges:

                        -Set/comb-out                                      $ 8.00

                        -Shampoo/set/comb-out                       $ 8.50

                        -Cut                                                     $ 6.00

                        -Permanent                                           $32.00

                        (Includes haircut, perm, set, & comb-out)

 

IN-HOME RESPITE BY PERSONAL CARE AIDE

• One-time registration fee                                     $  20.00
• In-Home Respite per hour                                  $  16.50
• 12-hour night care                                              $115.00
• 35 cents a mile travel reimbursement will be assessed to any client who lives 10 miles or more from Sunshine Terrace Adult Day Center.
