TERRACE FOUNDATION

We focus on our clients and strive to promote
WELLNESS of body, mind and spirit. We have a "can do" attitude.
Our clients and their families are the head of our interdisciplinary team---they
are in the driver's seat, we are their professional advisers.

We are in our 58th year as
a non-profit, community sponsored health care service.

We serve over 250 people per day, and have over 370 employees. 13.5% of our clientele is under the age of
70 with 4.5% under age 50.

We provide subacute, short term rehabilitative,
skilled, extended, respite, and specialized dementia care services in our Joint
Commission accredited nursing center, and in-home personal services, overnight respite
and adult day services from our CARF accredited adult day center, as well as assisted living
services at Terrace Grove, our assisted living center, which is Joint
Commission Accredited.

We are certified to provide Medicare and Medicaid
services, contracted with the Veteran's Administration and managed care
organizations, and provide uncompensated care to individuals who have no
source of reimbursement. We are licensed by the Utah Department of Health
for our nursing and assisted living center services, and by the Utah Department
of Human Services to provide adult day services. We have an excellent regulatory
compliance history, and earned a perfect score on our Joint
Commission Survey.

We are active members of the Utah Health Care
Association (state affiliate of the American Health Care Association),
and the American Health Care Association. CEO is an active member
of the American College of Health Care Administrators.

Our administrative costs are 6.78%.

We had over 75,000 hours of volunteerism in 2005,
and the community contributed over $260,450. We provided over
$1,364,000 in state defined uncompensated care. The
support of our community is a powerful key to our ability to deliver excellence
in care, service, and outcomes. We focus on delivering VALUE to our
clients and their families---quality, access to needed services, cost-effectiveness.
