PRIVATE CHARGE: $147.00/day PRIVATE ROOM: $168/day

RATE INCLUDES: (billed in advance, like rent)

· Room, bath

· Meals—any diet available, no extra charge—snacks

· Walkers, wheelchairs, canes for use here

· Services of rehabilitation aids (RNS)

· RN, LPN, nursing assistant care

· Personal laundry

· Activities, recreational outgoings

· Registered dietician consultation

· Basic Cable T.V. hook-up

· Social services consultation, discharge planning

· Assistance with filing insurance claims, filing for Medicaid eligibility both medically and financially

· Champlain services are requested

· Music Therapy services

CHARGES NOT INCLUDED IN DAILY RATE: (billed in arrears after use)

· Transportation

Round trip within 4 miles - $10.00 ($5.00 each way)

Round trip 4-10 miles $12.50

Round trip 10-22 miles $15.00

Round trip over 22 miles $20.00 ($10.00 each way)
